When completing the form, please make sure that you enter the correct email addresses in the correct fields. One is Personal email (for internal MAPS use only) – this is where we (MAPS) will communicate with you privately. The other, Clinic email – is what we will list in our clinician directory for parents to communicate with your clinic. We encourage you to submit a professional profile photo as well below.
